BEctel claims that a connection between brackets inappropriately between a climbing system and a concrete depot that was built caused the accident that killed three employees in April in a GNL export terminal project in Port Arthur, Texas.
The announcement of the suspicious cause is presented as part of a wider comment on the incident of Craig Albert, corporate president of Bechthel and Paul Marsden, president of his energy unit. From the accident, Bechtel officials have discussed the tragedy in terms of a greater effort that the soul is looking for about their security practices.
He was killed in the accident, Felix Jose Lopez, 42; Felipe Mendez, 25; and Reginald Magee, 41. López and Mendez declared themselves dead at the scene. Magee was taken to a local hospital where he died from his injuries. Two other workers injured in the accident were treated and they are expected to be completely recovered, according to authorities.
“The overwhelming sense of shock and sorrow we felt at that time has not faded,” the two said in a prepared statement.
The technical information published today is unusual, as the final investigation of occupational safety and health of the United States is not yet public, and there are two state demands pending contractors by families of workers and for survivors by accident.
In at least one lawsuit, Bechtel demanded that the applicants participate in an arbitration on claims as required in the employment agreements of the workers.
The result of Osha’s investigation and possible job safety are usually released six months after an accident.
Sempra infrastructure, based in San Diego, who awarded Bechtel’s contract the EPC project, owns and also accused in lawsuits.
What happened to the site is known in general terms. Three workers from the site fell approximately 65 feet to 85 feet in which it was described as a “incident of the tank jumping system” after a layer of scaffolding, part of the jumping shape system, connected to a concrete tank.
In a consultation in Bechtel, Enr asked if his staff controlled the formwork and his connections between brackets or if he considered other businessmen to be guilty, a key point that could affect the results of the demand. Enr also requested images of the bankruptcy connection itself, if something available that could be published. The company said he could not comment or provide the requested image.

But Bechtel provided a more detailed narrative about how he saw the events of the message of Albert and Marsden.
He said that the formwork jump is to lift the multi -plate formwork system and the work platform and to restore it above the tank to tie the reinforcement steel and place the next elevation of the concrete wall.
The members of the crew “had successfully jumped and set eight sections during their turn and set the last section at the time of the incident,” said Bechtel.
The company stated that eThe Ach formwork board has two vertical brackets, each connected to a cone that fixes the panel on the tank wall.
“Our research determined that while establishing the final section, the connection between brackets was not safely stuck in the cone screw, a steel screw form with an integrated washing machine,” Bechtel said, as seen in the photo at the top of the article. The cone screw is screwed into an addition [shown in green]which is launched on the concrete wall of the tank, added the firm.
