BEctel is reviewing his security program after an internal investigation into the Mortal Triple on April 29 at a Port Arthur, Texas Megaproject, where three employees fell to his death, the company announced last week in what he said it was a promised report on this probe.

He was killed in the accident, Felix Jose Lopez, 42; Felipe Mendez, 25; and Reginald Magee, 41. Lopez and Mendez declared themselves dead on the scene. Magee was taken to a local hospital where he died from his injuries. Two other workers injured in the accident were treated and they are expected to be completely recovered, according to authorities.

In this accident, as explained in an announcement of the report internally circulated on October 7 and published on the Bechtel website on October 8, the president of the company and the head of operations CRAIG Albert and the President of the Bechthel Energy Unit, Paul Marsden, say that the five -member crew included the newcomers of the relative place that lacked sufficient formation system in the system. Although the Three workers who died He had given training in protection against falls, “it seemed that they did not have their arrest of fall” linked to “an approved anchor point,” says Bechtel’s note. The company also did not assume new hires in a system of colleagues as required by its own policy, according to the executives.

“One of the most important efforts of our review is the role that the culture of security played and, in some cases, did not play,” say Albert and Marsden. “When reviewing the incident, we found breakdowns in supervision and supervision, where times were lost to intervene and take corrective actions.”

Executives also emphasize the need to “strengthen the proactive identification of critical risks for life by work crews and first -line supervisors during daily work planning”, as well as the need to “strict adherence to life -critical security processes, encouraging real -time conditions.

Bechtel New Rules

New security measures will be applied to the formwork jump and all work in Heights, the note establishes more.

According to his new rules, Bechtel Triple will verify that the connections between brackets are made correctly before jumping concrete forms, according to Albert and Marsden. The company said that a connection made inappropriately triggered the sudden loss of support and inclination of the work platform to the formwork of the Texas project.

But a key part of the company’s revelations involves the training of staff and culture of the company that seem short.

The company also assures that temporary work platforms are managed in the same way as traditional scaffolding and adding training at the level of work responsible for the workforce, supervision of security staff and acclaimed procedures on existing security protocols links.

The accident involved relatively recent hires that worked at night, which had successfully completed eight previous steps of formwork jump. But an experienced job manager and the crew hand had been called to help them elsewhere just before the accident, according to the executives.

The answers varied to the unusual release of a Bechtel internal report before the results of the U.S. Health Administration research were made public. One of the safety professionals says that information can be of great value to alerting other employers to build possible dangers.

Travis Livermore, a Dallas -based lawyer to Marshall Presley & Pipal, It states the publication of the information. “Bechtel is integrating the work authority into his routine, so there would be no stigma associated with the exercise, and so he is exercised, not only in the face of immediate danger, but also as a safeguard,” he says.

The contractor has also been “positioned” to negotiate the final potential citations of the OSHA, as contractors must report appointments to customers and potential clients, according to Livermore.

Legal implications

Livermore claims that in these negotiations an entrepreneur can defend himself, saying that a mortality was the result of an unforgettive action of employees. This works if the employer can show that he had established the rules of work, he has reported them, took steps to find rape and enforce the rules.

However, other aspects of Bechtel’s report ascend to My fault. The contractor points out, for example, that the crew had no training for the system used that night.

“”The five members of the crew were recent hires that, at the time of the hiring, had a previous experience limited or not in repositioning the feces jumping, ” Bechtel’s report states. “Although they had completed our general drop protection training and received training and experience at work and experiencing the jump formulas during the shifts before the incident, they had not received the 30 -minute classroom training prepared for supervision for the formwork jump system they used.”

Future high -risk work will be done during the day, as far as possible, Bechtel’s executives are committed.

Recognizing another deficiency, they say that the company will now automatically abstain the staff who violate their critical life rules, including the protection of falls, but suspend the discipline if the rape is “identified and corrected within the crew.” If not, the violations will be specific, including the possible completion.

Bracket insured inappropriately

In his investigation, Bechtel concludes that a connection between brackets inappropriately between the climbing form and a concrete depot that was built caused the accident. The work was part of an Estimated $ 13 million GNL export terminal project.

“The aim of sharing our findings is to promote transparency, accountability and learning, helping Bechtel – and the broadest industry – to provide risks and act in a proactive manner in security,” say Albert and Marsden.

But his note also contains a resignation that indicates that the report “not assigning legal responsibility or accounts for final conclusions” that may evolve after more hand data or “third party reports”.
